Due to the technical concepts of these systems exposure of the laryngeal structures is usually better than with direct laryngoscopy, both in normal airways as well as in those that are difficult to manage. With the increasing use of video laryngoscopy it seems as if direct laryngoscopy and flexible fibrescopic intubation are at risk of becoming redundant. In the 1940s, the technique of intraosseous infusion became established as an effective and safe means of delivering drugs, fluids and blood products. While the international guidelines for paediatric emergency medical care over the last two decades have accorded intraosseous infusion a high priority in this field, the technique is increasingly being advocated as an option for particularly difficult vascular access in the areas paediatric anaesthesia and perioperative care. This development is being furthered by the availability of modern, simple and safe canulation systems. According to the current Guidelines for Emergency Medical Care of the European Resuscitation Council intraosseous infusion is indicated whenever access to the circulation is imperative and a maximum of three cannulation attempts have failed (e.g. in cardiorespiratory arrest, acute haemodynamic instability, etc.). Additional indications include prolonged laryngospasm and massive airway bleeding in a child with no established iv-line, induction of anaesthesia in a child with a full stomach (rapid sequence induction) and failed cannulation. Semi-elective use of intraosseous infusion in non-critically ill children, such as in failed peripheral venous access after inhalational induction of anaesthesia should remain an option for individual cases only. Successful utilisation of the intraosseous infusion technique in paediatric emergency medical care and anaesthesia requires immediate availability of the equipment, regular training in its use, and unequivocal guidelines for its application in the specific medical setting). The updated guidelines of the European Resuscitation Council on life-saving measures for children (pediatric life support) and newborns (newborn life support) take these needs into consideration. New scientific findings were evaluated based on the evidence and the guidelines were simplified as far as possible in view of didactic feasibility. To improve the outcome of resuscitated children, prevention and early detection of critically ill children deserve particular attention. Recognizing the early phase of circulatory and respiratory insufficiency takes high priority since timely interventions are very effective and can be life-saving. This results in high stress levels for the provider and may trigger unsafe actions. Hence, a controlled induction technique (RSI-controlled) that involves gentle mask ventilation until full non-depolarizing muscular blockade has become increasingly popular. Clinical observation suggests that RSI-controlled may reduce the adverse effects noted above. We aimed to evaluate both techniques with respect to unsafe actions and stress. Methods In this controlled, randomized simulator-based study, 30 male trainees and specialists in anaesthesiology performed a simulated anaesthesia induction in a 4-week-old infant with pyloric stenosis. Two different RSI techniques, classic and controlled, were applied to 15 candidates each. We recorded the incidence of hypoxaemia, forced mask ventilation, and intubation difficulties. In addition, we measured individual stress levels by ergospirometry, salivary cortisol, and alpha-amylase, as well as a post-trial questionnaire. Results Hypoxaemia always occurred in RSI-classic but not in RSI-controlled, repeatedly resulting in unsafe actions. Subjective stress perception and some objective stress levels were lower in the volunteers performing RSI-controlled. Since the 1940s the technique of intraosseous infusion has established itself as a valuable alternative means for rapid, efficient and safe delivery of drugs and fluids to critically ill children. Whereas international guidelines for paediatric emergency medical care have assigned intraosseous infusion a high priority, most anaesthetists utilize this well-proven technique with great reluctance. This article describes the technique of intraosseous infusion, introduces two different cannulation systems, and discusses its potential indications in paediatric anaesthesia, based on current emergency medical care guidelines as well as some of our own case studies. In particular, children with acutely life-threatening conditions, such as circulatory arrest, laryngospasm, acute airway haemorrhage, hypovolaemic shock or hypothermia secondary to extensive burns, should receive an intraosseous cannula if intravenous access cannot be rapidly established. Future discussion may reveal whether a transiently inserted intraosseous infusion would also be indicated if the child with difficult or impossible venous access presents without acute life-threatening conditions for anaesthesia.
