Dadurch gewinnt die Palliativversorgung dieser vulnerablen Patientengruppe zunehmend an Bedeutung. Vor diesem Hintergrund wurde ein Curriculum zur Inhouse-Schulung palliativer Behandlung und Begleitung von Menschen mit Behinderung in einer Wohngruppe entwickelt, implementiert und evaluiert. Methode Vor (t1) und 6 Monate (t2) nach zehn Inhouse-Schulungsterminen à 90 Minuten wurde neben der Erfassung teilnehmerbezogener Daten der Lernzuwachs mittels Fragebögen mit offenen und geschlossenen Fragen erfasst. Themenschwerpunkte waren: Umgang mit Sterbe- und anderen belastenden Situationen, Herausforderungen im Berufsalltag und versorgungsbezogene Sicherheit. Die Angaben wurden deskriptiv statistisch und inhaltsanalytisch analysiert. Ergebnisse An der Schulung nahmen 16 Mitarbeiter teil; 14 füllten den Bogen zum Zeitpunkt t1 aus, von 9 Befragungsteilnehmern konnten t1- und t2-Angaben gewonnen werden. Schulungskonzept, Durchführung und mittelfristige Auswirkungen wurden dabei durchgehend positiv evaluiert; die Mitarbeiter gewannen in der Betreuung sterbender Bewohner und bei der Erfassung und Behandlung von belastenden Symptomen an Handlungssicherheit. Schlussfolgerung Ein Grundverständnis palliativen Handelns kann in Form einer Inhouse-Schulung alltagskonform und wirkungsvoll vermittelt werden, sodass sich die Handlungssicherheit der Mitarbeiter im Umgang mit schwer kranken, sterbenden und verstorbenen Bewohnern erhöht."],["dc.description.abstract","Background Life expectancy of people suffering from complex (mental and physical) disabilities has improved over the years. Therefore, palliative care for this particularly vulnerable group of patients is gaining importance. An educational curriculum for staff caring for severely disabled people living in a specialized residential home was developed, implemented and evaluated. Methods Before (t1) and 6 months after (t2) ten in-house 90-minute lasting educational sessions, perspectives of participants and knowledge gain was assessed using open and closed questions. Coping with dying and other stressful situations, day-to-day challenges, confidence regarding different aspects of end-of-life care, and other topics were explored. Data were processed by descriptive statistics and content analysis. Results Sixteen staff members attended the course; 14 answered the first questionnaire (t1). Follow-up data (t1 and t2) were available from 9 participants. Curricular aspects, performance and medium-term implications were evaluated as positive. Staff members described a gain in confidence in caring for dying residents and in assessing and relieving burdensome symptoms. Staff at a university hospital took part in a survey (January to June 2010) using a questionnaire with open and closed questions. Descriptive data for physicians and nurses (response rate = 13.5%, n = 101) are presented. Physicians and nurses reported similar high frequencies of ethical conflicts but rated the relevance of ethical issues differently. Nurses stated ethical issues as less important to physicians than to themselves. Ethical conflicts were mostly discussed with staff from one's own profession. Respondents predominantly expected the Clinical Ethics Committee to provide competent support. Mostly, nurses feared it might have no influence on clinical practice. Findings suggest that experiences of ethical conflicts might reflect interprofessional communication patterns. Expectations and objections against Clinical Ethics Committees were multifaceted, and should be overcome by providing sufficient information.
