Objectives. To assess the frequency of late reactions to reduced PPD patch test concentrations. Methods. In 1838 patients, PPD was tested at three concentrations (0.5% pet., group I; 0.4% pet., group II; and 0.35% pet., group III). Patch tests were read on D1 (D2) to D3 (D4); additional late readings were performed on D7, D14, and D21. Patients who were not able to return for all scheduled late readings were telephoned on D7, D14, and D21, and questioned about a reaction at the patch test sites. Results. Data of 1666 patients (1069 women and 597 men) were eligible. Late reactions were observed in 9 patients, 3 in group I (0.49%) and 5 in group II (0.63%). In 7 of 8 of the patients with late reactions, patch tests were applied for 48 hr. On retesting, 4 of 5 patients became positive at D2 or D3. Conclusions. The occurrence of late reactions to PPD may be influenced by patch test concentration and duration. The history includes the general information and specific data concerning used drugs, the classification and circumstances of the reaction. Skin tests are performed in all hypersensitivity reactions with allergic symptoms. Tests should be done between four weeks and six months after clearance of the symptoms by performing skin prick test, intradermal test, patch test or photopatch test. Validated tests for the detection of specific IgE antibodies in the serum are available for only few drugs, especially betalactam antibiotics. Other laboratory tests, e.g., the basophil activation test are done only in special cases. Provocation tests are indicated, if the culprit drug cannot be identified by the above mentioned tests. If possible, the evaluation of provocation tests should rely on objective parameters. Clinical manifestations range from urticaria to angioedema, rhino-conjunctivitis, bronchial asthma and anaphylactic shock. Methods: The aim of this study was to investigate the efficacy and safety of a 12-month latex-specific immunotherapy in sensitized patients, most often health care workers. Twenty-three patients with latex rhinoconjunctivitis (20 of whom also had asthma) were included in this randomized, double-blind, placebo-controlled trial (11 in the active group, 12 in the placebo group). Treatment efficacy was assessed by means of symptom and medication scores. Conjunctival provocation tests and quantitative skin prick tests were also performed. Results: The clinical index (derived by combining changes from baseline of six efficacy variables during the treatment period) did not differ significantly between treatment groups. Change from baseline of rhinitis, conjunctivitis, skin symptoms, asthma symptoms, medication score and cutaneous reactivity were not significantly different between the two groups. A nonsignificant difference in conjunctival reactivity was observed in favor of the active group (p = 0.09). Systemic reactions were much higher in the specific immunotherapy than in the placebo group. Conclusions: The present study failed to show a significant improvement of symptoms and medication scores, probably because of the low level of symptoms at baseline and the low maintenance dose of therapy, even if allergen-specific conjunctival reactivity decreased in the active group. Moreover, the incidence of systemic reactions was very high in the active group. Copyright (c) 2006 S. A dermatologist and allergist had diagnosed multiple food allergies by bioresonance technology, but neglected to perform skin tests. Following the doctor's recommendation, the patient avoided gradually more and more foods. Actually, her diet consisted of a certain kind of spelt bread, bananas, potatoes, pasta, butter, egg and salt. During the diet, she lost more than 10 kg of weight. The skin disease did not improve. The patient refused treatment with topical corticosteroids due to possible side effects. She reported an exacerbation of her dermatitis following the application of prednicarbate. Therefore, oral therapy with prednisolone was done. This improved the dermatitis. After completion, the eczema worsened again. The patient is otherwise healthy with no signs of atopic disposition and no other allergies. We performed patch tests for the first time in this patient. This revealed an allergic contact dermatitis mainly due to her cosmetics with positive reactions to methylchloroisothiazolinone/methylisothiazolinone (MCI/MI), fragrances and prednicarbate. Bioresonance is an allegedly diagnostic and therapeutical method invented and introduced to the market in the late 1970s. It is popular in alternative medicine in many, especially German-speaking, countries. Bioresonance is a controversial subject. Scientific studies did not show any effects other than placebo.
