Lockdown). Die vorliegende Arbeit soll klären, welche Auswirkungen diese Maßnahmen auf Rettungsdiensteinsätze hatten. Material und Methoden Retrospektive Auswertung von 6668 Einsatzprotokollen von vier Rettungswachen in Ostniedersachsen der beiden ersten Quartale 2020. Deskription und teststatistischer Vergleich der Einsätze sechs Wochen vor den Kontaktbeschränkungen mit einem gleich großen Zeitraum nach deren Erlass. Ergebnisse In den sechs Wochen im Lockdown gab es 17,7 % weniger Einsätze als in den Wochen vor dem Lockdown. Insbesondere zeigte sich eine Abnahme von Einsätzen wegen Atemwegserkrankungen um 40,6 % (91 Fälle), die insbesondere auf den Rückgang von Pneumonien und exazerbierten chronisch-obstruktiven Lungenerkrankungen (COPD) zurückgeht. Gleichzeitig zeigte sich ein Anstieg des durchschnittlichen Alters der Patienten mit einer Verringerung des Anteils der unter 65-Jährigen. Veränderungen bei psychiatrischen Erkrankungen, verstorbenen oder verletzten Patienten oder der Verweigerung von Behandlung und Transport wurden nicht beobachtet. Insgesamt wurden im Zeitraum 67 Patienten (1,0 %) mit Verdacht auf oder bestätigter COVID-19-Erkrankung behandelt. Diskussion Im Rettungsdienst zeigt sich eine Reduktion der Einsätze in Folge der Kontaktbeschränkungen, wobei diese nicht so stark ausfällt, wie für Notaufnahmen beschrieben wurde. Dieser Rückgang könnte auf eine Reduktion insbesondere weniger schwerer Erkrankungsfälle und jüngerer Patienten zurückzuführen sein. Auffällig ist die Reduktion von Pneumonien und exazerbierter COPD. Dies könnte einerseits bedeuten, dass Kontaktbeschränkungen das Infektionsgeschehen bei anderen Atemwegserkrankungen reduziert haben, aber ebenfalls, dass Patienten Krankenhausbehandlungen vermeiden wollten."],["dc.description.abstract","Background As a response to the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic, extensive contact restrictions were imposed by law in Germany as in other European countries. The present study intends to clarify the effect of these restrictions on emergency medical service (EMS) operations. Materials and methods Retrospective chart review of EMS operation protocols over the first 6 months of 2020 ( n  = 6668 rescue missions) in four rescue stations in eastern Lower Saxony (Germany). Description and statistical comparison of operations 6 weeks before the restrictions with an equally long period after the order of the restrictions (“lockdown”). Results During the 6 weeks after the lockdown the frequency of rescue operations decreased by 17.7%. In particular, there was a 40.6% ( n  = 91) decrease of emergency cases with respiratory diseases, mainly due to a decline of pneumonia and exacerbated chronic–obstructive pulmonary disease (COPD). At the same time, patients’ mean age increased with fewer patients under 65 years. There were no changes in the frequency of psychiatric disorders, deceased or injured patients, or refusal of treatment and transport. A total of 67 patients with suspected or confirmed SARS-CoV‑2 infection (1.0%) were observed during this period. Discussion EMS experienced a reduction of operations as a result of contact restrictions, although not as pronounced as was recently described for emergency rooms. This supports the hypothesis that the reduction is particularly evident in less severe cases and in younger patients. The reduction in pneumonia and COPD cases is striking. It is a background paper and reference manual, providing advocacy of general practice/family medicine (GP/FM) in Europe. The Research Agenda summarizes the evidence relating to the core competencies and characteristics of the WONCA Europe definition of GP/FM, and its implications for general practitioners/family doctors, researchers and policy makers. The European Journal of General Practice publishes a series of articles based on this document. In a first article, background, objectives, and methodology were discussed. In this second article, the results for the core competencies 'primary care management' and 'community orientation' are presented. Though there is a large body of research on various aspects of 'primary care management', it represents a very scattered rather than a meta view. Many studies focus on care for specific diseases, the primary/secondary care interface, or the implications of electronic patient records. Cost efficiency or process indicators of quality are current outcomes. Current literature on community orientation is mainly descriptive, and focuses on either care for specific diseases, or specific patient populations, or on the uptake of preventive services. Most papers correspond poorly to the WONCA concept. The influence of patient characteristics on family practitioners' (FPs') diagnostic decision making has mainly been investigated using indirect methods such as vignettes or questionnaires. Direct observation-borrowed from social and cultural anthropology-may be an alternative method for describing FPs' real-life behavior and may help in gaining insight into how FPs diagnose respiratory tract infections, which are frequent in primary care. Objective. To clarify FPs' diagnostic processes when treating patients suffering from symptoms of respiratory tract infection. Methods. This direct observation study was performed in 30 family practices using a checklist for patient complaints, history taking, physical examination, and diagnoses. The influence of patients' symptoms and complaints on the FPs' physical examination and diagnosis was calculated by logistic regression analyses. Dummy variables based on combinations of symptoms and complaints were constructed and tested against saturated (full) and backward regression models. Results. In total, 273 patients (median age 37 years, 51% women) were included. The median number of symptoms described was 4 per patient, and most information was provided at the patients' own initiative. Multiple logistic regression analysis showed a strong association between patients' complaints and the physical examination. Frequent diagnoses were upper respiratory tract infection (URTI)/common cold (43%), bronchitis (26%), sinusitis (12%), and tonsillitis (11%). There were no significant statistical differences between \"simple heuristic'' models and saturated regression models in the diagnoses of bronchitis, sinusitis, and tonsillitis, indicating that simple heuristics are probably used by the FPs, whereas \"URTI/common cold'' was better explained by the full model. Conclusion. FPs tended to make their diagnosis based on a few patient symptoms and a limited physical examination. Simple heuristic models were almost as powerful in explaining most diagnoses as saturated models. Direct observation allowed for the study of decision making under real conditions, yielding both quantitative data and \"qualitative'' information about the FPs' performance. Physicians need to consider them to arrive at a holistic treatment plan. Yet, it has not been systematically investigated as to which personal burdens ensue from certain health conditions. Objective: The objective of this study is to examine older patients' perceived burden of their health problems. Patients and methods: The study presents a cross-sectional analysis in 74 German general practices; 836 patients, 72 years and older (mean 79 +/- 4.4), rated the burden of each health problem disclosed by a comprehensive geriatric assessment. Patients rated each burden using three components: importance, emotional impact, and impact on daily activities. Cluster analyses were performed to define patterns in the rating of these components of burden. In a multilevel logistic regression analysis, independent factors that predict high and low burden were explored. Results: Patients had a median of eleven health problems and rated the burden of altogether 8,900 health problems. Four clusters provided a good clustering structure. Two clusters describe a high burden, and a further two, a low burden. Patients attributed a high burden to social and psychological health problems (especially being a caregiver: odds ratio [OR] 10.4, 95% confidence interval [CI] 4.4-24.4), to specific symptoms (eg, claudication: OR 2.3, 95% CI 1.3-4.0; pain: OR 2.3, 95% CI 1.6-3.1), and physical disabilities. Patients rated a comparatively low burden for most of their medical findings, for cognitive impairment, and lifestyle issues (eg, hypertension: OR 0.2, 95% CI 0.2-0.3). Conclusion: The patients experienced a relatively greater burden for physical disabilities, mood, or social issues than for diseases themselves.
