Increasingly more and differentiated options for this are becoming available in intensive care medicine. Within the framework of professional responsibility physicians must decide which of the available treatment options are indicated. This process of decision-making is determined by answering the following question: when and under which circumstances is induction or continuation of intensive care treatment justified? In addition to the indications, the advance directive of the patient is the deciding factor. Medical indications represent a scientifically based estimation that a therapeutic measure is suitable in order to achieve a defined therapy target with a given probability. The ascertainment of the patient directive is achieved in a graded process depending on the state of consciousness of the patient. We investigate the nature of the long-period radial velocity variations in a Tau first reported over 20 yr ago. Methods. We analyzed precise stellar radial velocity measurements for a Tau spanning over 30 yr. An examination of the Ha and Ca II lambda 8662 spectral lines, and HIPPARCOS photometry was also done to help discern the nature of the long-period radial velocity variations. Results. Our radial velocity data show that the long-period, low amplitude radial velocity variations are long-lived and coherent. Furthermore, Ha equivalent width measurements and HIPPARCOS photometry show no significant variations with this period. Another investigation of this star established that there was no variability in the spectral line shapes with the radial velocity period. An orbital solution results in a period of P = 628.96 +/- 0.90 d, eccentricity, e = 0.10 +/- 0.05, and a radial velocity amplitude, K = 142.1 +/- 7.2 m s(-1) Evolutionary tracks yield a stellar mass of 1.13 +/- 0.11 M-circle dot, which corresponds to a minimum companion mass of 6.47 +/- 0.53 M-Jup with an orbital semi-major axis of a = 1.46 +/- 0.27 AU. After removing the orbital motion of the companion, an additional period of approximate to 520 d is found in the radial velocity data, but only in some time spans. A similar period is found in the variations in the equivalent width of H alpha and Ca II. Variations at one-third of this period are also found in the spectral line bisector measurements. The similar to 520 d period is interpreted as the rotation modulation by stellar surface structure. Its presence, however, may not be long-lived, and it only appears in epochs of the radial velocity data separated by similar to 10 yr. This might be due to an activity cycle. Conclusions. Recently, we could show that RLX also induces MMPs in breast cancer cells and enhances in vitro invasiveness. To investigate its potential role for progression of breast cancer in vivo, RLX serum concentrations were determined in 160 breast cancer patients during post-surgical follow-up. RLX concentrations in cancer patients were significantly higher than in a control population of healthy blood donors and patients with various other diseases (0.47 versus 0.29 ng/ml, p < 0.0001). There was a significant difference between patients with metastases (0.62 ng/ml) and those without (0.38 ng/ml, p < 0.0001). Overall survival was shorter in RLX-positive (>0.4 ng/ml) than in RLX-negative patients (p = 0.016). Cox regression analysis showed that RLX was not an independent variable, in contrast to metastatic disease and primary lymph node involvement.
