In th is study overweight women did lose an average of 1.3 kg per year. The proportion of very successful participants (>= 5 % below initial weight) was about 20 %. If the programme was supported by a portable nutrition computer (mealus), an average of 2.0 kg per year was lost. In this group the proportion of very successful participants (>= 5 % below initial weight) was about 30 %. The evaluation algorithm was developed on the basis of a representative survey of the German population from 14 years of age. The classification was analogous to the well known intelligence quotients, which makes the results easier to understand. The questionnaire is available free of charge in the Internet (www.ernaehrungs-iq.de), so that it can be used by wide sections of the population, including both nutrition professionals and consumers. Both questions from older survey instruments and current questions are included. The results of the evaluation of a representative sample show that the nutritional knowledge of the German population is really good in some areas, but rather deficient in some aspects, particularly in practical questions. A comparison between the current results and identical or similar questions in older studies clearly shows that the knowledge of the German population in some areas - such as food labelling or energy requirements and consumption - has hardly improved during the last 10 or 20 years. Food labels such as \"Fett I. Tr.\" [dry weight fat] or the difference between fruit juice and fruit nectar are still difficult to understand for many consumers. It would be very interesting for the consumer if he could be provided with a correct, reliable and comprehensible product declaration, accompanied by explanatory information. But it should be remembered that decisions on food are only partially controlled by information and knowledge. Design: Prospective multicenter observational study in obese individuals undergoing a medically supervised outpatient-based 52-week treatment in 37 centers in Germany. Subjects: A total of 8296 participants with a body mass index (BMI) of 430 kg m(-2) included within 8.5 years. Measurements: Main outcome measures were body weight loss, waist circumference (WC), blood pressure, quality of life and adverse events. Results: In females, initial body weight was reduced after the 1-year-intervention by 19.6 kg (95% confidence intervals 19.2-19.9 kg) and in males by 26.0 kg (25.2-26.8) according to per protocol analysis of 4850 individuals. Intention-to-treat (ITT) analysis revealed a weight reduction of 15.2 kg (14.9-15.6) in females and 19.4 kg (18.7-20.1) in males. Overall, the intervention resulted in mean reduction in WC of 11 cm; it reduced the prevalence of the metabolic syndrome by 50% and the frequency of hypertension from 47 to 29% of all participants (ITT, all P<0.001). The beneficial effects could be documented for up to 3 years and comprised significant improvement of health-related quality of life. The incidence of adverse effects was low; the only event repeatedly observed and possibly related to either the intervention or the underlying disease was biliary disorders. Conclusion: The present non-surgical intervention program is a highly effective treatment of obesity grades I-III and obesity-related diseases, and therefore, could be a valuable basis for future weight maintenance strategies required for sustained success. According to the most recent guidelines of the German Obesity Society, reducing fat intake is the first-line strategy in the dietary treatment of overweight and obesity. Compliance is a main problem of self-help programmes. Without external controls, many participants soon give up. The present study has shown that a handheld nutrition computer clearly improves compliance; the quota of those who give up is reduced by 70 %. The computer was especially useful during those weeks in which self monitoring was required. The therapeutical success of the programme could probably be further improved if the computer is not only used in 5 out of 12 weeks, but over the entire programme period. It might be difficult for populations with other eating habits to follow such diets. Therefore, a modified Mediterranean-type diet using fat modification through neutral and butter-flavored canola oil, walnuts and walnut oil with two portion-controlled sweet daily snacks was tested in Germany. SUBJECTS/METHODS: Randomized waiting-list control study with overweight/grade 1 obese subjects: 12-week self-help modified Mediterranean-type diet, 6 weeks of diet plans and 6 weeks of weight loss maintenance training. Trial duration was 12 months. Intervention group (IG) included 100 participants (average age of 52.4 years, weight 85.1 kg and body mass index (BMI) 30.1 kg/m(2)), waiting-list control group (CG) included 112 participants (52.6 years, 84.1 kg and 30.1 kg/m(2)). RESULTS: Per-protocol weight loss after 12 weeks was 5.2 kg in IG vs 0.4 kg in CG (P <= 0.0001), BMI - 1.8 vs - 0.1 kg/m(2) (P <= 0.0001), waist circumference -4.7 vs -0.9 cm (P <= 0.0001). Triglycerides, total cholesterol and LDL cholesterol improved significantly in IG but not in CG. One-year dropouts: 44% in IG and 53% in CG. Weight loss after 12 months: 4.2 kg (pooled data). CONCLUSION: A five-meal modified Mediterranean-type diet with two daily portion-controlled sweet snacks was effective for weight management in a self-help setting for overweight and grade 1 obese subjects.
