However, brain exposure to high cortisol concentrations may also accelerate disease progression and cognitive decline. The objectives of this study were to investigate whether HPA-axis dysregulation occurs at early clinical stages of AD and whether plasma and CSF cortisol levels are associated with clinical disease progression. Morning plasma and CSF cortisol concentrations were obtained from the subjects with AD dementia, mild cognitive impairment of AD type (MCI-AD), MCI of other type (MCI-O), and controls with normal cognition included in a multicenter study from the German Dementia Competence Network. A clinical and neuropsychological follow-up was performed in a subgroup of participants with MCI-AD, MCI-O, and AD dementia. CSF cortisol concentrations were increased in the subjects with AD dementia or MCI-AD compared with subjects with MCI-O or normal cognition. After controlling for possible confounders including CSF measures of amyloid beta1–42 and total tau, higher baseline CSF cortisol levels were associated with faster clinical worsening and cognitive decline in MCI-AD. This cross-sectional study investigated the association and interplay between musical instrument playing during life, multi-domain cognitive abilities and brain morphology in older adults (OA) from the DZNE-Longitudinal Cognitive Impairment and Dementia Study (DELCODE) study. Participants reporting having played a musical instrument across three life periods (\r\n n\r\n = 70) were compared to controls without a history of musical instrument playing (\r\n n\r\n = 70), well-matched for reserve proxies of education, intelligence, socioeconomic status and physical activity. Participants with musical activity outperformed controls in global cognition, working memory, executive functions, language, and visuospatial abilities, with no effects seen for learning and memory. The musically active group had greater gray matter volume in the somatosensory area, but did not differ from controls in higher-order frontal, temporal, or hippocampal volumes. However, the association between gray matter volume in distributed frontal-to-temporal regions and cognitive abilities was enhanced in participants with musical activity compared to controls. We show that playing a musical instrument during life relates to better late-life cognitive abilities and greater brain capacities in OA. Musical activity may serve as a multimodal enrichment strategy that could help preserve cognitive and brain health in late life. We examined these groups vs healthy controls (HC) to reveal HippSub alterations between diseases. Methods: We segmented 3T-MRI T2-weighted hippocampal images of 67 HC, 58 BD, and MDD patients from the AFFDIS study and 137 patients from the DELCODE study assessing cognitive decline, including subjective cognitive decline (SCD), amnestic mild cognitive impairment (aMCI), and AD, via Free Surfer 6.0 to compare volumes across groups. Results: Groups differed significantly in several HippSub volumes, particularly between patients with AD and mood disorders. In comparison to HC, significant lower volumes appear in aMCI and AD groups in specific subfields. Smaller volumes in the left presubiculum are detected in aMCI and AD patients, differing from the BD group. A significant linear regression is seen between left hippocampus volume and duration since the first depressive episode. Conclusions: HippSub volume alterations were observed in AD, but not in early-onset MDD and BD, reinforcing the notion of different neural mechanisms in hippocampal degeneration. Die diagnostischen Kosten hingegen verursachen\n davon nur einen geringen Teil. Mit Wandel des konzeptuellen\n Verständnisses von Demenzerkrankungen, der\n Behandlungsmöglichkeiten und der Leitlinien spielen zunehmend auch\n Biomarker-Untersuchungen eine wichtige Rolle. Die ökonomischen\n Auswirkungen der Biomarker-basierten Diagnostik sind derzeit nicht sicher\n abschätzbar. Zur Erhebung der Kosten einer leitlinien-orientierten\n ätiologischen Erst-Diagnostik von kognitiven Störungen wurde\n eine Umfrage im Deutschen Netzwerk Gedächtnisambulanzen (DNG)\n durchgeführt. An 15 Expertenzentren des DNG wurden systematisch die\n Personalbindungszeiten für alle Prozeduren und alle beteiligten\n Berufsgruppen erhoben und die Personalkosten basierend auf den\n tarifvertraglichen Arbeitgeberkosten berechnet. Zusammen mit den Kosten\n für technische Untersuchungen wurden Gesamtkosten der Diagnostik\n für drei Szenarien abgeschätzt: Diagnostik ohne Biomarker\n € 633,97 €, Diagnostik mit Liquoruntersuchungen\n € 1.214,90 und Diagnostik mit FDG- plus Amyloid-PET\n € 4.740,58. Zusätzlich erfolgte eine Analyse der\n derzeitigen realen Kostensituation in Gedächtnisambulanzen, wobei die\n Personalbindungszeiten für einzelne Leistungen und die apparativen\n Kosten ins Verhältnis zur Häufigkeit ihrer Anwendung gesetzt\n wurden. Als Mittelwert aller Zentren ergeben sich dabei Gesamtkosten von\n € 1.394,43/Fall (Mittelwert der Personalkosten\n € 351,72, Mittelwert der Kosten für apparative\n Diagnostik € 1.042,71). Die Ergebnisse zeigen, dass eine\n ätiologische Diagnostik von kognitiven Störungen (Demenzen und\n leichte kognitive Störung) einen Ressourceneinsatz erfordert, welcher\n derzeit weder durch die Vergütungssysteme von Ambulanzen noch durch die\n vertragsärztliche Vergütung kostendeckend erstattet wird. Die\n Biomarker-gestützte Diagnostik dementieller und prädementieller\n Syndrome wird häufiger werden, wenn sie zur Indikationsstellung vor\n einer krankheits-modifizierenden Therapie erforderlich ist.
